Getting It So Wrong

A woman with a history of breast cancer goes to her Doctor complaining of shoulder pain. He sends her for an X-ray and the radiologist says it reveals a cancerous tumour. But her GP "forgets" to mention this crucial information. The woman later dies for the disease. Should the Doctor be named and shamed?
